Our Process

Our Care Process

We believe in a thoughtful, step‑by‑step approach that helps families feel informed and supported from the very first conversation. Our process is designed to provide clarity, continuity, and care that adapts as needs change.

01

Step 1: Understanding Your Needs

We begin by learning about your situation, priorities, and independent living goals. This initial conversation helps us understand what support is needed and how we can best assist you or your loved one.

02

Step 2: Personalized Care Plan® Development

Using our client-focused systems built through 20+ years of supporting Vancouver Island families, we create a Personalized Care Plan® that outlines the right level of care for health, safety, and peace of mind.

03

Step 3: Consistent Caregiver Matching

Through our Consistent Care Approach®, clients are paired with familiar caregivers whenever possible. This continuity supports trust, comfort, and both emotional and physical well-being.

04

Step 4: Customizing Services

Care plans are customized using a wide range of available services, including assisted transportation, short-term and post-operative care, in-home foot care, respite care, companion care, healthy meal preparation and planning, dementia care, live-in care, handyman support, and downsizing assistance.

05

Step 5: Heal-at-Home Recovery Plan® (When Needed)

For clients recovering from surgery or hospitalization, we implement the Heal-at-Home Recovery Plan®, designed to support faster, more complete healing in the comfort of home.

06

Step 6: Daily Living & Recovery Support

Caregivers assist with everyday tasks such as meal preparation, light housekeeping, grocery shopping, transportation to medical appointments, medication reminders, and support with rehabilitation exercises, helping reduce stress and physical strain.

07

Step 7: Confident Transportation Advantage®

Our Confident Transportation Advantage® provides dependable transportation using dedicated drivers and wheelchair-equipped vehicles, supporting independence and reducing missed appointments.

08

Step 8: Ongoing Communication

We maintain regular communication with clients and families to ensure care remains aligned with needs and expectations.

09

Step 9: Care Plan Adjustments

As needs change, services and schedules are adjusted to continue providing appropriate support at every stage.

10

Step 10: Continued Guidance & Support

Whether care is short-term or ongoing, we remain a consistent point of contact, offering guidance and coordination to support long-term independence and well-being.
